Call to Order Meeting was called to order by Larry Landman at 4:50pm.
Environmental Health Jeff discussed the Environmental Health statistics. Currently they have done 52 septic systems, and Jeff is projecting about 20 more in the spring.
Jeff presented to the Board an Interagency Agreement. This is an agreement between Lyon County and the other counties in Region 3. The agreement is for mutual aide in the event of loss of staff for illness, vacations, ect. He questioned whether this is something Lyon County would be interested in. The Board of Health is requesting some updates be made to the Contract. The updates are as follows: need to add Lyon County under the first paragraph, under #1 need to add Lyon County and also show #1b as this was missing on the document brought to board, under #5 change the dates to reflect current dates, add back in all of #7 as part of that paragraph was also missing. It was felt by the Board of Health once the above items are added back into the contract Larry could sign the contract.
Jeff presented his budget for Environmental Health to the board. It is attached in the Board of Health minutes. Jeff also noted currently they have $20,000.00 in a fund account through the courthouse that could be used for well closings if the grant would be expended.
Jeff attended classes recently at the Iowa Onsite Wastewater Organization. He attended classes to maintain his Time of Transfer Certification.
Jeff also purchased a TruCore Sludge Sampler to aid in doing Septic Tank samples. This Sampler will take a sample of the entire depth of a septic tank. This will give them the different levels of waste in a septic tank.
Jeff also discussed Radon testing. He stated an EPA rating of 4 or below is safe. He noted to the Board that Northwest Iowa has some of the highest radon levels in the country. He is trying to get more testing done in Northwest Iowa. He did talk with the Board about the possibility of applying for a 50/50 grant through the Grand Falls Casino. This would be a grant used along with the homeowner to install an evacuation system where 50% of the cost would be the homeowners, and 50% would come from the grant. Radon testing kits are $6.00 and are distributed through the Lyon County Engineers office.
Motion by Carol and seconded by Bonnie to have Larry sign the contract after the listed changes have been made to the contract. Motion carried unanimously.
Motion by Bonnie seconded by Carol to approve the Environmental Health budget as presented. Motion carried unanimously.
Agenda Approve Agenda
Sherri requested an addition be made to the Agenda. She would like to add under Health Promotion / Risk Reduction to approve the lead paint violation notice.
A motion was made by Bonnie and seconded by Carol to approve the agenda for the January 18, 2012 meeting, with the addition to the agenda. Motion carried unanimously

A motion was made by Carol and seconded by Bonnie to approve the minutes from the November 9, 2011 meeting. Motion carried unanimously
Election of Officers Discussion on the annual election of officials. Larry remaining as the Chairperson and Dr. DeJong as the Vice Chair. Nomination made by Bonnie, seconded by Carol, to have Larry Landman remain as the Chairperson on the Board of Health. Motion carried unanimously.

Annual Paperwork Larry signed the Delegation of Signatory Authority, and Administrative Personnel Policy. Sherri will bring the Standing Orders for Immunizations to Dr. DeJong to sign. All members present signed the Conflict of Interest paperwork.
Financial / Administrative
Report Sherri reviewed and discussed with the Board the revenue and expense sheets for the month ending December 31, 2011.
Sherri noted a few of the expense accounts. She stated Health Supplies and Equipment is higher than projected due to Influenza Vaccine all has been purchased already for the year. Technical Services is quite low due to not having to use the interpreter much this year. Mileage is a little higher, this was originally budgeted at $0.45/mile but this year the rate went up to $0.48/mile. Office Equipment, in this account was a new computer for Melissa Stillson, and Sherri was able to bill that purchase to the PHEP Grant.
Sherri presented the proposed budget for FY 12-13. She is requesting a 3.5% increase in wages. She did go over her budget by line and what is included in each line. Her budget is attached.
She did figure the IPERS on the budget presented at 8.07% and it should be at 8.67%. She is going to reflect that change in the budget that goes to the Board of Supervisors.
Sherri discussed the Program Evaluation Annual Report and gave all members a copy of that report. She briefly went over the report and the statistics. A copy of the Program Evaluation Annual Report is attached.
Mariannette Miller-Meeks, the Director of Iowa Department of Public Health, came to Health Services of Lyon County on January 6, 2012. Sherri invited the members of the Board of Supervisors and the Board of Health. Dr. Miller-Meeks was able to meet with the staff of Health Services of Lyon County as well as two members from the Board of Supervisors.
Sherri received in the mail a document about joining NALBOH, which is the National Association of Local Board of Health. Sherri was wondering if this is something the Board of Health would be interested in joining. It was felt by the Board to not join at this time.
Motion by Bonnie and seconded by Carol to approve the proposed budget with the 3.5% increase in wages, and the change to the percentage for IPERS. Motion carried unanimously.
Home Care Sherri presented November and December statistics. Visits have been up over the last two months. Home Care has also received a higher number of referrals. Chris has started a referral log to show statistics on how many referrals are received, how many are admitted to the program, and the time it takes on each referral.
The Medicare Provider Revalidation has been received by our office and Melissa is currently working on that. Medicare is requiring a fee to be revalidated. The fee for calendar year 2012 is $523.00.
Healthy Families Sherri read a summary of services, for the past two months, written by Melissa Stillson. Currently Amy is at 11 families and Melissa is at 13 families. In November there were 44 visits. In December there were 41 visits.
Melissa and Amy did a parent group event on Nutrition.
Immunization/ Communicable Disease Sherri discussed the Regional Epi report for Lyon County for August through December. Sherri briefly went over the statistics on Epi cases that have been identified in Lyon County during that time period.
IDPH is exploring the idea to mandate a TDaP immunization for all High School age students. IDPH would like to see all High School age students have at least one dose of the Pertussis.
IDPH has set up a site visit for the VFC Immunization Program. They are planning to come on January 31, 2012.
Health Promotion / Risk Reduction Sherri received a copy of the letter Sioux County will be sending to homes that were previously identified with high lead levels, from the Board of Health, advising them of the need to rid their home of lead and what consequences they may have if this is not done. Sherri explained that Julia Schroeder will first send out a letter from IDPH advising of the changes needed to rid their home of lead paint. If she is unable to get the homeowner to do the necessary updates, she would like this letter to send out to the homeowner. Prior to the Board meeting, Sherri did forward a copy of that letter to Carl Petersen, Lyon County Attorney to look over. She did hear back from Carl that it would be okay to send out the letter. Sherri is asking if our Board of Health would be interested in sending out a letter from the Board of Health to the homeowner. The Board of Health was in agreement to approve the letter with a change in name reflecting Lyon County Board of Health. Motion by Carol and seconded by Bonnie to revise the Sioux County letter to reflect Lyon County and send that letter to the homeowners, if nothing has been done after the IDPH letter was sent out. Motion carried unanimously.
Emergency Planning Sherri has been talking with Wayne Jepsen, county coordinator for the Emergency Preparedness Plan. In the county plan Lyon County Health Services portion is called ESF8. Sherri would like to add ESF8 as an attachment to the Annex A currently in the Health Services of Lyon County Preparedness Plan. The Board thought it was okay to add the ESF8 as an attachment in Annex A of the Emergency Preparedness Plan.
Sherri also went over the yearly updates to each Annex. Annex A was updated with new 24/7 contacts, and Community Direction and Control. Annex B was updated with the HAN policy, and reviewed additional attachment to reflect the current Board of Health Members. Annex C was updated with the EPI training for the staff and who has all taken that training, and the change to the hospital administrator. Annex D was updated with the Lyon County Medical Professional listing from the State. Annex E was updated to show the critical supply vendors in an emergency, and a list of Region 3 emergency managers and sign language interpreters. Annex F had no changes. Annex G was updated to change name of cell phone service. Annex H was updated with an additional form called the demobilization check out form, and the vaccine adverse event reporting system. Sherri is required to update and list the changes that have been made to the Preparedness Plan, and have Larry sign that the updates have been reviewed by the Board of Health. Larry did sign this document and it will be placed in the emergency plan.
The Mid-year report for the PHEP workplan is done and submitted. Sherri will still need to schedule a workshop and tabletop exercise to complete the requirements of the Preparedness Grant.
School The surveys and projected costs have been sent out to the schools.
Schedule The next Board of Health meeting is scheduled for Wednesday, March 14, 2012 in the conference room at the Lyon County Annex.

Financial / Administrative
Report Joanne reviewed and discussed with the Board the revenue and expense sheets for the month ending December 31, 2011.
Joanne presented the proposed budget for FY 12-13. She is requesting a 3.5% increase in wages. She is also requesting her salary be based at 38.5 hours per week instead of 38 hours per week. There will also be an increase to fringe as one more employee has signed up for Health Insurance compared to the last fiscal year. She did go over her budget by line with the Board of Health. Her budget is attached.
Motion by Bonnie and seconded by Carol to approve the budget with the 3.5% wage increase, and the increase from 38 to 38.5 hours for Joanne’s salary hours per week.
Program Statistics Joanne presented the second quarter statistics. October started with 62 clients and since the beginning of October there have been 13 admissions and 13 discharges. At the end of December there were 62 clients remaining. During this quarter there were 1511.75 total Homemaker and Home Care Aide Hours. This is slightly lower than last quarter.
Joanne presented her Annual Program Evaluation to the Board. Everyone was given a copy and a copy attached in the Board of Health notes. Larry did question the county contribution to the Homemaker program. Joanne will get that information for the next Board of Health meeting. She will also add that to her Annual Evaluation to be included in future evaluations.
Misc. Items Joanne reported on the workability of the injured Aide. The Aide is still on restrictions on what she is able to do for 4 more weeks. At that time they will evaluate and see what restrictions if any are needed.
Joanne talked with the Board of Health regarding a policy on Verification of OIG Excluded Individuals search. This is something that will be checked every month to check the status of employees, and any contracted employees, and the agency as a whole. This is done for verification that we have not collected Medicare or Medicaid dollars on a claim that was performed by an excluded individual. Joanne is going to add this policy to her policy book.

LYON COUNTY HEALTH
SERVICES PROGRAMS

HOME CARE
Health Services of Lyon
County provides skilled nursing care and physical therapy
in your home. Home health care must be prescribed by
your physician and is provided by a variety of health care
professionals.
All services are provided on
a flexible basis in an attempt to meet individual needs,
and services are available to all residents of Lyon
County. This Agency is certified with Medicare and
Medicaid and also utilizes private insurance with home
care coverage if appropriate. Services not covered
by third party payment will be charged privately according
to a sliding fee schedule.

HOMEMAKER
The homemaker program places
trained and supervised aides in the home of families and
adults for a temporary or extended period of time, because
of illness or disability. Components of this service
include: personal care, medication supervision, meal
preparation, limited housekeeping, essential shopping,
meal and respite care.
School Health
Health Services of Lyon
County contracts with public and parochial schools within
Lyon County to provide: direct classroom education,
screenings, coordination of health care plans on students
with chronic illness and to serve as a consultant on
health care issues.
IMMUNIZATION
The goal of the immunization
program is to reduce and ultimately eliminate the
incidence of vaccine preventable diseases by working in
conjunction with the public and private health care
providers throughout the state. Accomplishing this
goal will require achieving and maintaining high
vaccination coverage levels, improving vaccination
strategies among under vaccinated populations, prompt
reporting and thorough investigation of suspected cases,
and rapid institution of disease control measures.
Health Services of Lyon
County offers immunizations to all children and adults by
appointment.

HEALTHY FAMILIES PROGRAM
Healthy Families is a
family-centered, strength-based program that provides in-home
support and assistance to parents of young children, prenatal
through five years of age that reside in Lyon County.
The goals of our program are to
support and strengthen families in bulding the foundation of
safe, nurturing environments in which children thrive.
Healthy Families is a voluntary program offered at no cost
through Northwest Iowa Empowerment Funding.
HEALTH PROMOTION
Health Services of Lyon County
participates in a four county Childhood Lead Poisoning
Prevention Program through a grant from the Iowa Department of
Public Health. Our goal is to have every child under the
age of six tested for lead poisoning. Our focus is on
increasing awareness of lead poisoning and testing, inspecting
houses where lead-poisoning children live, and assisting in the
care of lead poisoned children and their parents.

LYON CO. TOBACCO COMMUNITY PARTNERSHIP
Lyon County Tobacco Community
Partnership is funded through a grant form the Iowa Department
of Public Health.. Our goals are to 1) prevent tobacco use
initiation among youth 2) promote cessation among young people
and adults 3) eliminate nonsmokers' exposure to secondhand smoke
4) identify and eliminate the disparities related to tobacco use
and its effect among different population groups. Lyon
County is teaming up with Sioux County to form Sioux/Lyon
ASH-Action on Smoking and Health.
EMERGENCY PLANNING and PREPAREDNESS
Health Services of Lyon County's
role in emergency planning and preparedness is to provide an
organized and comprehensive response to a bio-emergency or a
terrorist attack involving a biological agent.
