Agenda A motion was made by Bonnie and seconded by Anita to approve the agenda for the July 14, 2010 meeting. Motion carried unanimously. Minutes A motion was made by Anita and seconded by Bonnie to approve the minutes from the May 19, 2010 meeting. Motion carried unanimously. Environmental Health Jeff was unable to attend the Board of Health meeting. Information presented by Sherri. An Environmental Health meeting was held for training for Board of Health and Board of Supervisor members. Larry and Jeff were able to attend this meeting. Jeff has given out two loan interest applications, one septic application in June, five septic applications in July, and has received one complaint of illegal discharging of septic waste. Guest Speakers Diane Anderson, Regional Community Health Consultant presented on Chapter 137 of Iowa Code. The language was changed in Chapter 137 to make it possible to form a district Board of Health if the Local Board of Health would choose to do this. They included the legal documentation necessary to form a district board. Sherri has a copy of this information and will send a copy out to the board members. Diane also presented on the Local Board of Health toolkit. It is an online resource center on all type of subjects related to the Board of Health. Sherri also has the website for this and will send on to each member of the Board of Health. Financial/ Administrative Report Sherri reviewed and discussed with the Board the expenditure/revenue reports for the month ending June 30, 2010. She did ask for a $3,000.00 budget amendment from the supervisors and was approved. She ended the year with $5,683.24 remaining on her expense budget. She discussed the line items that were over budget on the expense side. She also presented the revenue’s and we were $86,020.24 over budget. CHNA/HIP Community Health Needs Assessment / Health Improvement Plan. Every five years the IDPH requires Local Boards of Health to hold meetings to discuss and address community health needs. Sherri has held one meeting among staff of Health Services, where they brainstormed to get a list of agencies to include in the meeting. They have come up with 31 different agencies to participate. She will send letters to these agencies asking for their participation. Once she receives the responses from the people who are participating she will set up meetings to discuss what people currently feel is a community health need. Once the group narrows this down to the top need, Sherri will write up a plan and submit it to the state. Iowa Healthy Initiative NWICC is currently in the process of constructing a rec area, fitness center, and community room. They would like to apply for a Harkin Grant. They may ask for a letter of support from Lyon County Board of Health. Sherri is currently attending the meetings for the Iowa Health Communities Initiative and will update the board if a letter of support is needed. Board is in agreement to send letter if it is needed. Home Care Sherri presented the home care statistics. She compared this years to last years statistics. D&D visits for this year 1095, and last 1036. Therapy visits this year 173, and last 196. HCA visits this year 2096, and last 1926. This year we had 39 procedure visits and 13 evaluation visits. Sherri discussed the pros and cons for the IAHC membership. She has decided to pay the yearly membership. Communicable Disease We had only one case of mumps in Lyon County during the recent Northwest Iowa outbreak. To date there have been no cases of West Nile reported in Lyon County. We have taken some phone calls regarding questions about West Nile. We have also developed a packet of information for staff to use when taking these phone calls Emergency Planning Sherri went over the Agency Emergency Response Plan, POD plan, and Pan Flu plan that Heath Services has. She informed the Board of the changes she has made. She updated the HAN policy/procedure. She can have 5 HAN users who receive a call or email depending on the level of alert. In June we had our performance measure where we received a HAN alert that tested our call down tree. She updated the PIO from Sherri to Joanne. She changed any attachments with Kandi’s name on them to now reflect her name. She also went over all the chapters included in the Emergency Response Plan. She also discussed the Point of Distribution Plan (POD Plan). She needs to have the Board of Health sign off that they reviewed the updates to these plans this year. Motion by Chet and seconded by Anita to sign the emergency plan. Motion carried unanimously. School Health No program changes for FY2011. All evaluations for school nurse program were good. Healthy Families Sherri discussed the yearly Healthy Families statistics. They were down to 44 families this year compared to 50 from last year. She did point out that over the last year there were 18 families discharged with 11 of them being discharged after completing the program. She also discussed the visit/level schedule for the clients. They currently have the levels set up on a goals process. This year the Healthy Families program did have to turn back some of their Empowerment funds as they were not used in the Fiscal year. Chet asked how they are going to continue to get referrals since the hospital is no longer doing deliveries. We have spoken with Joe at Sanford Clinic Rock Rapids and they will hand out the Healthy Families pamphlet two times during the course of a pregnancy. We have also spoken to Dr. Wierda who comes to Sanford Clinic Rock Rapids to see OB patients. She was going to talk to her case workers about the program. Empowerment this year has changed their name to Northwest Early Childhood Iowa. Health Promotion Lead Coalition - Sherri has made one Lead visit to a family of a child with an elevated lead level to discuss healthy habits. Tobacco Coalition – They have hired a new Tobacco director. Sherri will be attending both the tobacco coalition and lead coalition meetings tomorrow in Sioux County. The next Board of Health meeting is scheduled for Wednesday, September 15, 2010 in the conference room at the Lyon County Annex. Joanne discussed the year end expenses that she was over budget on, but as a total remained under the budget for expenses. She also went over the revenue side and her program ended $19,767.55 over budget on revenue. Statistics Joanne presented the year end statistics. The homemaker program finished with 67 clients. They had 33 admissions and 31 discharges. They also had a total of 6,599.5 hours of service time. Misc Items/Discussion Vacation time has caused some shuffling of Aides. They have all been really helpful in picking up the extra time. Joanne is working on the United Fund application. Last year the program received $975.00. Joanne is working on Aide evaluations. She will be doing them in July. She will be working on her program evaluation in July as well. The next Board of Health meeting is scheduled for Wednesday, September 15, 2010 in the conference room at the Lyon County Annex. LYON COUNTY HEALTH SERVICES PROGRAMS

HOME CARE

 

Health Services of Lyon County provides skilled nursing care and physical therapy in your home.  Home health care must be prescribed by your physician and is provided by a variety of health care professionals.

 

All services are provided on a flexible basis in an attempt to meet individual needs, and services are available to all residents of Lyon County.  This Agency is certified with Medicare and Medicaid and also utilizes private insurance with home care coverage if appropriate.  Services not covered by third party payment will be charged privately according to a sliding fee schedule.

 

 

HOMEMAKER

 

The homemaker program places trained and supervised aides in the home of families and adults for a temporary or extended period of time, because of illness or disability.  Components of this service include: personal care, medication supervision, meal preparation, limited housekeeping, essential shopping, meal and respite care.

 

School Health

 

Health Services of Lyon County contracts with public and parochial schools within Lyon County to provide: direct classroom education, screenings, coordination of health care plans on students with chronic illness and to serve as a consultant on health care issues.

 

IMMUNIZATION

 

The goal of the immunization program is to reduce and ultimately eliminate the incidence of vaccine preventable diseases by working in conjunction with the public and private health care providers throughout the state.  Accomplishing this goal will require achieving and maintaining high vaccination coverage levels, improving vaccination strategies among under vaccinated populations, prompt reporting and thorough investigation of suspected cases, and rapid institution of disease control measures. 

 

Health Services of Lyon County offers immunizations to all children and adults by appointment.

 

 

 

HEALTHY FAMILIES PROGRAM

 

Healthy Families is a family-centered, strength-based program that provides in-home support and assistance to parents of young children, prenatal through five years of age that reside in Lyon County.

 

The goals of our program are to support and strengthen families in bulding the foundation of safe, nurturing environments in which children thrive.  Healthy Families is a voluntary program offered at no cost through Northwest Iowa Empowerment Funding.

 

HEALTH PROMOTION

 

Health Services of Lyon County participates in a four county Childhood Lead Poisoning Prevention Program through a grant from the Iowa Department of Public Health.  Our goal is to have every child under the age of six tested for lead poisoning.  Our focus is on increasing awareness of lead poisoning and testing, inspecting houses where lead-poisoning children live, and assisting in the care of lead poisoned children and their parents.

 

 

LYON CO. TOBACCO COMMUNITY PARTNERSHIP

 

Lyon County Tobacco Community Partnership is funded through a grant form the Iowa Department of Public Health..  Our goals are to 1) prevent tobacco use initiation among youth 2) promote cessation among young people and adults 3) eliminate nonsmokers' exposure to secondhand smoke 4) identify and eliminate the disparities related to tobacco use and its effect among different population groups.  Lyon County is teaming up with Sioux County to form Sioux/Lyon ASH-Action on Smoking and Health.

 

 

EMERGENCY PLANNING and PREPAREDNESS

 

Health Services of Lyon County's role in emergency planning and preparedness is to provide an organized and comprehensive response to a bio-emergency or a terrorist attack involving a biological agent.
