Meeting was called to order by chair Larry Landman at 5:15 p.m.
Minutes A motion was made by Kathy and seconded by Bonnie to approve the minutes from the March 25, 2009 meeting as presented. Motion carried unanimously.
Tobacco Prevention
Coordinator Kandi introduced Cheryl Buntsma, Tobacco
Prevention Coordinator for Lyon, Sioux,
O’Brien, Osceola, and Plymouth counties. She reviewed and discussed with the Board her role as the Tobacco Prevention Coordinator. She reviewed the Coalition goals, promotion cessation, preventing tobacco use, and maintaining a coalition. See attached. Each Board member also received her 3rd quarter newsletter for review.
Environmental Health Jeff Williams gave an update on the recent training he had attended. He reviewed and discussed with the Board the environmental activities regarding permits and wells. Lyon County is more
aware of radon testing. Radon test kits are available.
Jeff has also completed the training for Implementing and Sustaining Continuous
Quality Improvement in an Organization.

Administrative Report Kandi reviewed and discussed the expenditure and revenue summary reports

for month ending April 2009.
Kandi noted the LPHS grant alternative plan for FY 2009 was approved during the Homemaker/Home Care Aide Board meeting. Copy on file.
Kandi noted that Lyon County will receive $5,789.00 in Emergency Planning Carryover Funds.
The Board reviewed the letter received from the Regional Community Health Consultant regarding the Compliance audit. This audit will be placed on file.
Kandi reviewed the initial information regarding funding to IDPH from this legislative session. More information will be coming.
Home Care There has been an increase in referrals in the Home Care program.
Kandi reviewed the atatistics.
The Home Care program is partnered with Avera McKennan using PtCt. Currently they are looking at various programs that are available to replace this software.
Communicable Disease/
Immunization Kandi reviwed and discussed with the Board the future of the Immunization
program. The local medical clinic will soon become a VFC provider. We anticipate a decrease in numbers to our public clinic. We will be looking at other immunization opportunities perhaps with adolescent school based clinics.
Kandi gave an update on the H1N1 events that occurred. This was an opportunity
to work with our partnerships within Lyon County and at the State level.
Emergency Planning Kandi gave an update on our emergency planning exercises to be conducted on May 30, June 23, and June 25.

Kandi discussed with the Board our carryover dollars. The money is to be used for emergency preparedness. Kandi presented to the Board the plan for utilization of carryover funds for their review. The activities included H1N1 time, and staff exercise time. Supplies include a TV, wall mount, and cable hook-up to use for training and staying informed during a real time event. Other supplies include a laptop computer, case and portable printer, heavy extension cord and surge protector, fit testing kit for N95s, incident command vest and incident command board. Purchases will be made by June 30, 2009.
A motion was made by Bonnie and seconded by Anita to approve the FY09 Public
Health Plan for Utilization of Carryover Funds. Motion carried unanimously.
School Health Kandi gave a school update. The Board reviewed the quarterly job summary. Larry questioned the liability in the area of counseling/mental health. Kandi will look into this area.
Healthy Families Kandi gave an update.
Health Promotion Kandi gave an update.
The next Board of Health meeting is scheduled for Wednesday, July 15, 2009 at 4:45 p.m. in the conference room at the Lyon County Annex.
A motion was made by Anita and seconded by Bonnie to adjourn at 6:45 p.m. Motion carried unanimously.

Administrative Report Sherri reviewed and discussed the expenditure and revenue summary reports for the month ending April 2009.
Sherri reviewed and discussed with the Board the third quarter cost report.
Sherri discussed changes in the cost allocation for the director’s time on the cost report. The percentage of her time not spent in the homemaker program will be subtracted from her salary to determine the year end cost report for FY 2009.
Alternate Plan Sherri reviewed and discussed with the Board the Alternative Plan for FY09 for Local Public Health State Grant. The activity requested Home Care Aide (homemaker) be added to use $2,324.00.
A motion was made by Kathy and seconded by Anita to approve the Alternative Plan FY09 LPHSC. Motion carried unanimously.

SLT/Chore The Senior Living Trust and Chore Services grant program is being audited May, 21, 2009.
The next Board of Health meeting is scheduled for Wednesday, July 15, 2009 in the conference room at the Lyon County Annex.

LYON COUNTY HEALTH
SERVICES PROGRAMS

HOME CARE
Health Services of Lyon
County provides skilled nursing care and physical therapy
in your home. Home health care must be prescribed by
your physician and is provided by a variety of health care
professionals.
All services are provided on
a flexible basis in an attempt to meet individual needs,
and services are available to all residents of Lyon
County. This Agency is certified with Medicare and
Medicaid and also utilizes private insurance with home
care coverage if appropriate. Services not covered
by third party payment will be charged privately according
to a sliding fee schedule.

HOMEMAKER
The homemaker program places
trained and supervised aides in the home of families and
adults for a temporary or extended period of time, because
of illness or disability. Components of this service
include: personal care, medication supervision, meal
preparation, limited housekeeping, essential shopping,
meal and respite care.
School Health
Health Services of Lyon
County contracts with public and parochial schools within
Lyon County to provide: direct classroom education,
screenings, coordination of health care plans on students
with chronic illness and to serve as a consultant on
health care issues.
IMMUNIZATION
The goal of the immunization
program is to reduce and ultimately eliminate the
incidence of vaccine preventable diseases by working in
conjunction with the public and private health care
providers throughout the state. Accomplishing this
goal will require achieving and maintaining high
vaccination coverage levels, improving vaccination
strategies among under vaccinated populations, prompt
reporting and thorough investigation of suspected cases,
and rapid institution of disease control measures.
Health Services of Lyon
County offers immunizations to all children and adults by
appointment.

HEALTHY FAMILIES PROGRAM
Healthy Families is a
family-centered, strength-based program that provides in-home
support and assistance to parents of young children, prenatal
through five years of age that reside in Lyon County.
The goals of our program are to
support and strengthen families in bulding the foundation of
safe, nurturing environments in which children thrive.
Healthy Families is a voluntary program offered at no cost
through Northwest Iowa Empowerment Funding.
HEALTH PROMOTION
Health Services of Lyon County
participates in a four county Childhood Lead Poisoning
Prevention Program through a grant from the Iowa Department of
Public Health. Our goal is to have every child under the
age of six tested for lead poisoning. Our focus is on
increasing awareness of lead poisoning and testing, inspecting
houses where lead-poisoning children live, and assisting in the
care of lead poisoned children and their parents.

LYON CO. TOBACCO COMMUNITY PARTNERSHIP
Lyon County Tobacco Community
Partnership is funded through a grant form the Iowa Department
of Public Health.. Our goals are to 1) prevent tobacco use
initiation among youth 2) promote cessation among young people
and adults 3) eliminate nonsmokers' exposure to secondhand smoke
4) identify and eliminate the disparities related to tobacco use
and its effect among different population groups. Lyon
County is teaming up with Sioux County to form Sioux/Lyon
ASH-Action on Smoking and Health.
EMERGENCY PLANNING and PREPAREDNESS
Health Services of Lyon County's
role in emergency planning and preparedness is to provide an
organized and comprehensive response to a bio-emergency or a
terrorist attack involving a biological agent.
